I, , resident of , a doctor from and my registration # desire to be a part of Oxxy for digital consultation to patients so that more and more patients can benefit from my experience and advice.

I understand that this will give me immense exposure amongst the patients.

This online/chat/talk consultation will be a free service and I will not charge Oxxy or the patient for the consultation. However at a later stage if the patient comes back to me for any test, treatment or surgery, I can charge my dues from the patient. For this, I will not have to pay Oxxy any share.

I can guide patients the way I feel fit. Since the chat/talk will be only between the patient and me, I indemnify Oxxy for the outcome of my consultation to the patient.

I will be available for the patients 9am to 7pm on all working days. If by chance I miss any call, I promise to call them back within 4 hours. Also, I will be providing all the recordings with the patient to Oxxy on regular basis.

This MoU is valid for 5 years from this date and will keep on renewing automatically unless otherwise terminated in writing.
